Cancelling a dialysis booking |

It may be necessary for you to cancel your holiday haemodialysis due to changes in circumstances, i.e. accident or illness. As soon as you know your dialysis will need to be cancelled, ring the holiday dialysis unit and confirm in writing. |

Cancellation charges are calculated from the date of the notification of cancellation. When the written notice of cancellation is received the following amounts will be payable: |

· From time of booking up to 8 weeks before your dialysis is due to start - £50.00 booking fee |
|
· Between 3 and 8 weeks before your dialysis is due to start – 50% of the total dialysis cost |
|
· Less than 3 weeks before your dialysis is due to start – 100% of the total dialysis cost. |
|
· In the event of demise or transplantation full refund will be given. |

Cancellation on our part |
|
We try not to change or cancel your dialysis treatment. However we reserve the right to do so when it is absolutely necessary, or due to circumstances beyond our control. We will inform the hospital as soon as possible. |
